The Home Health After hours admission/On Call Nurse is a licensed RN (Registered Nurse)  who administers skilled nursing care in the home to clients requiring professional nursing service. These services are performed in collaboration with the other members of the home health multidisciplinary team, in accordance with the physician's orders and under the direction and supervision of the Clinical Supervisor.

• On call nurse will work: 
  • 4pm-7:30 am, Monday through Friday and every other weekend 
  • 2 Holidays per year 
• Required to see 3-5 admissions per week. 
• On call nurse will triage emergency phone calls from Home Care patients after hours. 
• On call nurse will make visits depending upon the need Skilled Nursing Care and Comprehensive Assessment Skills Teaching Supervision and Counseling Clients and Caregivers Client Care Coordination. 
• Documentation of Interdisciplinary Communication to assure necessary follow up with team in during the next business day. 
• Documentation Problem Solving and Critical Thinking Skills
•  Time Management 
• Development of Plan of Treatment 
• Other duties as assigned. 

Valid Drivers License in the State of Delaware with no more than 3 violations.

Prior Home Care nursing experience required.
