Job Overview

We are seeking compassionate and dedicated Home Health Physical Therapists (PTs) to join our growing team across Texas. This role offers the opportunity to deliver high-quality, patient-centered care in the comfort of patients’ homes while maintaining flexibility and autonomy in your schedule.

Key Responsibilities

• Evaluate and assess patients’ physical conditions and develop individualized treatment plans
• Provide skilled physical therapy services in a home health setting
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises, mobility techniques, and safety
• Maintain accurate and timely documentation in compliance with regulations
• Collaborate with physicians, nurses, and interdisciplinary team members

Qualifications

• Active Physical Therapist (PT) license in Texas
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Previous Home Health experience preferred (new grads with strong clinical skills may be considered)
• Strong communication and interpersonal skills
• Valid driver’s license and reliable transportation
